As we implement safety measures to ensure the health of our school community during the COVID-19 pandemic, we are reducing the amount of paper that goes home with students and returns to school. Normally, the documents below would have been sent home in hard copy. This year we ask that you complete the forms and scan or send a photo of each document to your child's teacher via e-mail
. We please ask that you attach all the documents in one e-mail. You can find teacher e-mail addresses by going to the "About
" page and clicking on the teacher's name under the Staff List.
There is one form that we cannot send electronically as it is specific to each child. This is the verification sheet. We will be sending this item home in hard copy with each child. If possible, please scan or take a photo of this sheet also. We appreciate your help in reducing the number of items that go back and forth from home to school and vice versa.
